Personalized care plans
Ordering of medication and medical supplies
Assistance with activities of daily living (ADLs), to include but not limited to eating, toileting, hygiene care, dressing, mobility, and behavior management.
We have experience with a range of mobility devices for residents who have limited mobility.
These include sit-to-stand lifts, Hoyer lifts, transfer poles, walkers, gait belts and canes. This is in collaboration with physical therapists to optimize level of function and ability.
We help manage care for residents with a variety of chronic illness. These include (but are not limited to):
Metabolic diseases (diabetes, metabolic syndrome)
Heart diseases (hypertension, angina, coronary artery disease, vascular disease, congestive heart failure)
Lung diseases (COPD, Asthma, obstructive sleep apnea- with use of CPAP)
Neurological diseases (stroke, multiple sclerosis, Parkinson’s, dementia, aphasia, epilepsy)
Cancer
Arthritis

Joint Replacement
Management of colostomy/urostomy
Management of feeding tubes
